Information we collect on this website
- Information you submit: when you complete our contact form we collect your name, email address, phone number, and your child's age range. We use it for one purpose: to contact you about scheduling a free clinical consultation call.
- Usage data: we may use analytics and advertising tools (Google Analytics, Meta Pixel) that collect standard device and usage information through cookies. You can control cookies through your browser settings and opt out of personalized advertising via aboutads.info/choices.
How we use your information
- To respond to your inquiry and schedule your consultation call.
- To operate, measure, and improve our website and advertising.

Health information
This website form is a contact request, not a medical intake. Please do not submit detailed health information through it. Once you become a patient, your health information is protected under HIPAA and handled only in HIPAA-compliant systems, as described in our Notice of Privacy Practices below.

Contact requests are retained only as long as needed to respond and manage scheduling. We use HTTPS on this site and restrict access to submitted information to authorized staff.

HIPAA — Notice of Privacy Practices (Summary)
For patients of Atlas Literacy & Language PLLC: we are required by law to maintain the privacy of your protected health information (PHI), to provide you notice of our legal duties and privacy practices, and to follow the terms of our Notice of Privacy Practices.
- We use and disclose PHI only for treatment, payment, and health care operations, or as otherwise permitted or required by law.
- You have the right to access and receive a copy of your records, request corrections, request restrictions on certain uses, and receive an accounting of disclosures.
- You have the right to file a complaint with us or with the U.S. Department of Health and Human Services without retaliation.

Good Faith Estimate — Your Rights (No Surprises Act)
Under the federal No Surprises Act, health care providers must give patients who don't have insurance or who are not using insurance a Good Faith Estimate (GFE) of the expected cost of services, in writing, before services are provided.
- You have the right to receive a GFE for the total expected cost of any non-emergency service, including evaluations and therapy sessions.
- Atlas provides a written GFE before your first billed service, and upon request at any time.
- If you receive a bill that is at least $400 more than your GFE, you can dispute the bill.
